This agreement ("Agreement") between You, hereinafter referred to as "Client," and Senior Moments Inc./Ginell Butler, hereinafter referred to as "Gerontologist/Consultant," concerning the representation of Client in the contractual agreement for a fee of $3000.
1. Scope of Representation:
Senior Moments Inc. agrees to provide the Client in the contract agreement, providing case management services:
- Social
- Geriatric
- Financial and Housing Resources
- Advice and Representation (as needed) throughout the case.
2. Fee Structure:
The Client agrees to pay a flat fee of $3000 for the case management. Services provided by Senior Moments; Inc. Payment shall be due upon the execution of this Agreement.
3. Additional Costs:
Any additional costs, extensions, including but not limited to fees, communication, filing fees, and other expenses related to the case management, shall be the responsibility of the Client.
4. Termination:
Either party may terminate this Agreement with written notice. In the event of termination, the Client shall be responsible for any unpaid fees for services rendered up to the termination date.
5. Confidentiality:
Senior Moments Inc. agrees to maintain the confidentiality of all information related to the Client's case and shall not disclose any information without the Client's consent, except as required by law.
6. Governing Law:
This Agreement shall be governed by and construed in accordance with the laws.
