- Graduating Spring 2026 from a high school in the Astera Health Service Area
- Pursuing a healthcare-related career
- Upload current transcript with GPA and acceptance letter
- Share your achievements, extracurriculars, community service, and philanthropy work
- Submit a short essay (300–500 words) on your healthcare career motivations
